Consent for personal data processing (“Questionnaire”)

I, being a natural person and filling in the “Questionnaire” on the websitehttps://www.ntt.su/ by means of the relevant form, acting voluntarily, according to my expressed will and in my own interests, and also acknowledging my full legal capacity, hereby give my consent for personal data processing (hereinafter – Consent) to the Operator – NTT Innovative Pipe Solutions (PSRN 5077746425257, TIN/TRRC 7707622256/504201001, e-mail address: info@ntt.su, phone: +7 (495) 151-11-27 (hereinafter – Operator, NTT LLC), which is the owner of the website https://www.ntt.su/ and is registered at the following address: 141320, Russia, Moscow Region, Sergievo-Posadsky Urban District, Kovrovo village 50, room 2.

Before giving this Consent to the Operator, I have read the Personal Data Processing Policy of the website https://www.ntt.su/, located at the address: https://www.ntt.su/politic, in compliance with which the Operator processes personal data.

The Consent is given on the following terms:

1. This Consent is given for personal data processing both with and without the use of automation means.

2. The Consent is given for processing of my personal data as follows:

• Full name;
• mobile phone number;
• e-mail address;
• company name.

3. Abovesaid personal data does not fall into any specialized categories of does not fall into any specialized categories and is not considered biometric personal data.

4. The aim of personal data processing: processing of information contained in the Questionnaire.

5. In the course of processing, the following actions will be performed concerning personal data: collection; recording; systematization; accumulation; storage; verification (update, alteration); extraction; use; transfer (provision, access); blocking; deletion; destruction.

6. Third parties processing personal data under order of NTT LLC for the aim specified in this Consent:

7. Personal data is processed within 30 days from the moment of completion of the discussion of the project based on the Questionnaire filled in and delivered to us, or from the moment of receipt of revocation of the Consent for personal data processing, whichever occurs first.

8. I understand that this Consent can be revoked by me or my representative by means of sending a request to NTT LLC in electronic form, signed in accordance with the legislation in the field of electronic signatures, to the address: info@ntt.su marked as “Revocation of consent for personal data processing”.

9. This Consent shall come into effect from the moment it is given and until the moment of termination of personal data processing in accordance with clause 7 hereof.

10. I understand that in case of revocation of the Consent by me or my representative, NTT LLC shall have the right to continue processing of my personal data without my Consent in case there are grounds as specified in paragraphs 2-11 of part 1 of article 6 of the Federal Law No. 152-FZ “Concerning personal data” of 27.07.2006.
